Positively Being Well Course Agreement

By participating in this class, I understand and agree to the following:

• I acknowledge that the class is for information, educational and relaxation purposes only. Nothing taught in this class is meant to replace medical treatment or consultation with a medical professional.
• I will not be certified or qualified to provide or teach massage to the public upon completion.
• I understand that massage/bodywork is not a substitute for medical examination, diagnosis, or treatment and that I should see a qualified medical specialist for any mental or physical ailment.
• The instructor is not responsible for the manner in which I choose to use the information learned.
• Because massage/bodywork should not be performed under certain medical conditions, I affirm that I have consulted with a medical provider prior to applying the techniques described. There shall be no liability on the instructor’s part should I fail to do so.
